Tiina Brown
Tiina Brown is a clinical pharmacist with a special interest in geriatric medicine, deprescribing and supporting safe transitions of care. She has experience in both community and hospital pharmacy, providing valuable insight into medication risk across care interfaces and strengthening continuity during transitions between settings. She works within multidisciplinary models, including Geriatric Evaluation and Management in the Home (GEMITH), where she supports safe medication optimisation for community-dwelling older people with complex needs. She also has a strong interest in improving clinical documentation quality to strengthen communication and handover across care settings, particularly between hospital and primary care.
